Statements are sent on or before the 22nd of each month. If not paid by end of business day the 10th, a $20.00 late fee will be incurred. By signing this application for service, the undersigned hereby grants to Blue Fiber Corp./Youngsville TV corporation a license, irrevocable for the duration of the membership, to come upon the property of the undersigned upon reasonable advance notice for the purpose of repairing, replacing, maintaining and/or inspecting any equipment owned or installed by Youngsville TV corporation.
